This form provides the opportunity for eligible candidates to apply for review and acceptance into the New Mexico Job ChalleNGe Academy. The application must be completed with accurate information to be eligible for review.

This document grants the NMJCA staff limited power of attorney to make decisions regarding your applicant's medical care, participation in program activities, and access to records. Required for program participation.
